Grievance Form Step 1 of 3 33% IHSTECA Grievance FormIn the case of a question in reference to selected athletes of All-District, All-State, and any other selected student athletes, the Indiana High School Tennis Coaches Association will honor the following grievance procedure. The head coach of the athlete in question will agree to the grievance and file all paperwork (Head Coach's Letter, Administration Letter, Player Record Sheet). All information must be received within 30 days after the official release is posted on our web page of the information in question. All three documents must be submitted by the HEAD COACH. This request must be presented on school stationary with the required signatures. Upon receipt of the grievance information, the IHSTeCA Board of Directors will review the grievance.Head Coach's Description:(Required)Max. file size: 128 MB. The head coach must explain the grievance in detail. This request must be presented on school stationary with the required head coach's signature.Administrative Support:(Required)Max. file size: 128 MB. The head coach must obtain support of the grievance from the athletic director and principal of the school the athlete is representing. This request must be presented on school stationary with the required signatures of both parties.Player Record Sheet:(Required)Max. file size: 128 MB. Attach the record sheet of the Singles Player or Doubles Team being discussed. Include all varsity results throughout the season. For any doubles matches, include the partner of the player being nominated. ALL Matches along with results must be present. Incomplete forms will make the Grievance null and void.
