All persons participating in or attending the Indiana Tennis Hall of Fame banquet, agree to indemnify and hold harmless the IHSTECA and ICGSA sponsoring the organizations and its members from any and all losses, claims, injuries (Including death), or damages, including all costs and expenses, which may occur from my attendance herein.
Any special condition of a participant or attendee must be communicated directly to the coordinating officer prior to the banquet.
Disclaimer (Coaches @ Convention):
If you are a High School Coach signed up for the 2026 Indiana High School Tennis Convention then you do not also have to sign up for the Hall of Fame Banquet through this form. Your attendance is already confirmed when you selected the Hall of Fame Dinner option as part of your convention fee. However you must sign up for the convention by January 19th, 2026 if you wish to attend the HOF Banquet.
